- Set a deadline for response. Approvals hanging in inboxes for weeks delay everything downstream.

Have a default action documented. 'If no response by X date, we'll proceed with default option Y' prevents indefinite waiting.
How formal should change orders be?
Always written, signed if material. Verbal approvals are how disputes start; signed change orders are how disputes get resolved.
