Honest enough to be actionable, kind enough not to be a HR liability. Anything you wouldn't say to their face shouldn't be in the doc.

Should I share the written review before the conversation?

Yes for self-reviews and ratings (so the conversation isn't a surprise). Keep some manager observations for the verbal discussion to leave room for response.
