How often should access be reviewed?

Quarterly for sensitive systems (financials, customer data, production environments). Annually minimum for everything else. Less than annual fails most compliance frameworks.

What if a manager doesn't respond to access reviews?

Default to revocation, with notice. The review exists to confirm continued need; silence is not the same as confirmation.
